At that time, the supply of eggs in the ovaries begins to deplete and the levels of estrogen, the primary female sex hormone, drop precipitously. What is not as well recognized is that something quite similar happens to men – termed andropause. We might instead call it MAN-opause.

During andropause, the production of sperm in the testicles decreases (although not to zero) and the levels of testosterone, the primary male sex hormone, also begin to drop. Andropause is a much more gradual process than menopause, and, since men do not have menstrual cycles (which obviously cease with menopause), it is not as obvious. Men might notice a decrease in energy levels, less muscle mass, lower strength, worsening sleep patterns, a negative change in their mood, decrease in their sex drive and reduction in their sexual functioning.

There are blood tests helpful in diagnosing andropause, usually a combination of low blood testosterone levels and a corresponding increase in pituitary hormones suggesting the brain is “unhappy” with the lower levels of male sex hormone. One problem that exists is that many physicians are not adept at interpreting the results of these tests.

When the low testosterone state is diagnosed, there are a variety of replacement strategies that can be used. These include topical creams, sprays and patches as well as intra-muscular injections and even implantable testosterone-releasing pellets. Once again, many physicians do not have the expertise necessary to help their patients choose the best form or the proficiency in interpreting later blood tests to determine whether the replacement strategy is indeed working.

Have Your Heard About The HCG Diet?

It was first designed in the early 1940s when an American physician began using the hormone Human Chorionic Gonadotropin (HCG) on young men with low testosterone levels. The physician soon noted that not only did HCG help with the young men’s low testosterone symptoms, but they also started losing significant weight, claiming to experience a dramatic decrease in appetite. It was determined that HCG decreased appetite by influencing the satiety center in the brain and also seemed to promote the breakdown of peripheral fat stores.

The traditional HCG Diet involved 23-42 days of injections of small amounts of HCG along with a very-low-calorie diet (VLCD) of about 500 calories per day, consisting of two small servings of lean protein, two large servings of vegetable and two small servings of fruit. The prevailing thought at the time the HCG Diet was developed was that eating fat made you fat.

What we have learned since that time is that eating fat is NOT what makes you fat, but that eating fructose (found in fruit) DOES. Also starting the day with only black coffee or tea is probably a bad idea in terms of slowing down body metabolism. Therefore, my own variant of the HCG Diet involves four small servings of lean protein and two large servings of vegetable with the breakfast meal consisting of one of the protein servings. I have had good results using this approach with most patents losing from 15 to 25 pounds on the 23 day program. Obviously this variant of the HCG Diet is only a starting point in terms of losing weight so that your future self will be much leaner and healthier.

High-Intensity Interval Training

weight loss naples flYou may have heard of High-Intensity Interval Training (HIIT). HIIT involves doing exercise at a high intensity for a specific period of time followed either by a period of rest or much lower level of intensity, then repeating the sequence. A typical HIIT workout might only last 15 to 20 minutes. HIIT has been shown to be extra helpful in promoting weight loss since, during HIIT, your body burns stored glycogen as a fuel source.

Traditional aerobic exercise involves doing a moderate level of intensity for a more prolonged period of time, which ends up burning stored fat. Most people assume that burning stored fat is what you want to do. But, by burning stored fat, you are sending a signal to your body that stored fat is what is required to fuel your exercise. So, you burn stored fat for your 30 to 60 minutes of moderate intensity exercise then your body restocks the fat the remainder of the day so as to be ready for tomorrow’s workout. This metabolic fact may explain why so many people you see on the treadmill at the gym will look the same tomorrow, next week, next month and next year as they do today.

When you do HIIT and burn stored glycogen, the remainder of the day is spent with your body restocking its glycogen stores to be ready for tomorrow’s workout. How does the body accomplish this? One way is by converting stored fat into stored glycogen. So, instead of burning stored fat for 30 to 60 minutes and restocking it for the remaining 23 to 23.5 hours of the day, you end up burning stored fat for 23.75 hours (assuming you do a 15 minutes HIIT workout).

Another way is by converting dietary carbohydrate from sugars and starches into glycogen, which is a storage form of carbohydrate found in skeletal muscle. Carbohydrate intake would otherwise lead to blood sugar spikes and thus blood insulin spikes which decrease metabolism, increase appetite and lead to belly fat deposition. Thus, HIIT ends up decreasing blood sugar and blood insulin levels, boosting daily metabolism, decreasing appetite and decreasing belly fat. Also, since glycogen is found within skeletal muscle, HIIT ends up further developing skeletal muscle and helps promote that “lean, ripped” look so many of us crave.

There are several ways to design a HIIT workout. One way is by distance. You could walk from one street light to the next, then jog from that street light to the next, then sprint from that street light to the next, then repeat the sequence. Another way is by time. You could do high intensity for 30 to 60 seconds then rest or do lower intensity for 15 to 30 seconds, then repeat the sequence. A final way is by intensity, best measured by heart rate (you obviously need a heart rate monitor to do HIIT this way).

You do high intensity to get your heart rate well above your target (144 for a person in their 40s, 136 for a person in their 50s, 128 for a person in their 60s and 120 for a person in their 70s) then do lower intensity until your heart rate falls back toward your target, then repeat the sequence. Please remember, before a HIIT (or any other) workout, do a warm-up for two to three minutes and do a cool-down for two to three minutes after the workout as well.

naples heart instituteCardiovascular disease involves the development of atherosclerosis, or “hardening of the arteries,” within the walls of the arterial portion of the cardiovascular system. The arteries are what supply oxygenated blood (from the lungs) to the remainder of the body. The pressure within the arteries is higher than that of other portions of the cardiovascular system (veins, capillaries and lymphatics). Due to this increased pressure, the arterial walls have rather prominent smooth muscle bands in their central portion, called the tunica media. The outer lining of the arterial wall is called the tunica adventitia while the inner lining is called the tunica intima. The inner “skin” of the arterial wall (in contact with the bloodstream) is called the endothelium.

Atherosclerotic cardiovascular disease involves damage and abnormal functioning of the endothelium and intima with the deposition of cholesterol and presence of inflammation within the intima. This eventually may lead to sudden cardiac death, heart attacks, strokes, ruptured aneurysms, heart failure, kidney failure, gangrene and amputations.

The injury of the endothelium and intima seems to be caused by various factors, those related to genetics and the aging process (termed “non-modifiable” risk factors) as well as those related to smoking and levels of blood cholesterol, blood pressure and blood sugar (termed “modifiable” risk factors). It used to be assumed that only the modifiable risk factors could be influenced (thus the name) but we have learned since the Human Genome Project was completed in 2003 (when all 25,000+ of the human genes were identified) that even though you can’t change your genes, it is possible to turn certain genes “on” and other genes “off.” Thus, at present, there are medical strategies available that are potentially able to modify ALL of the major risk factors of cardiovascular disease.
